We will do a complete evaluation of yours or your loved one's current healthcare, financial and insurance status and develop a plan of care based on their needs and wishes.
ELDERCARE ASSISTANCE
We will find the right services for your needs. We can arrange in-home care and manage household duties. We can also coordinate the selection of nursing, assisted or independent living facilities.
MEDICAL CARE COORDINATION
We understand medical care coordination is crucial to a client's emotional and physical well- being. Services include: Providing communication to healthcare providers and family members regarding each client's medical plan. Creating and supervising a medication schedule. Accompanying client to a physician or other medical appointments or attending facility care conferences and communicating outcomes. We can also make appropriate referrals to healthcare agencies.
FINANCIAL
Healthcare costs can be debilitating. Whether you are facing a current issue or want to be prepared, we can review your current assets and insurance to create a financial plan to meet your healthcare needs. VA and Medicaid eligibility requirements and application assistance.
LEGAL
We can review current or needed healthcare documents and make referrals to healthcare specialists including Elder Law and Estate Attorneys.
RELOCATION
Depending on the client's wishes we can assist with the coordination of moving from home to another care location including boxing up of personal belongings and arranging them in their healthcare community.
